I'm Dr. Jac Constantino, a Doctor of Nursing Practice and board-certified Psychiatric Mental Health Nurse Practitioner with additional board certification in Adult-Gerontology Primary Care. My background allows me to understand the close connection between mental and physical health, providing a comprehensive approach to psychiatric care that considers the whole person—not just a diagnosis.

With years of experience treating adults across a wide range of mental health conditions, I specialize in comprehensive psychiatric evaluations, thoughtful medication management, and individualized treatment planning. Initial Evaluation

A thoughtful, comprehensive assessment designed to understand your unique experiences and create a personalized treatment plan.

Your first appointment is a comprehensive psychiatric evaluation where we'll take the time to understand the full picture of your mental health. During this 90-minute visit, we'll discuss your current concerns, emotional well-being, medical and psychiatric history, previous treatments, medications, lifestyle, relationships, and personal goals. This in-depth assessment allows us to develop an accurate diagnosis and an individualized treatment plan tailored to your specific needs.

My approach is collaborative, compassionate, and evidence-based. You'll have the opportunity to ask questions, gain a better understanding of your symptoms, and actively participate in developing a treatment plan that supports your long-term mental wellness.

Personalized, evidence-based follow-up care focused on helping you achieve lasting emotional wellness.

During your follow-up appointment, we'll review your progress, discuss how you're responding to treatment, evaluate medication effectiveness and any side effects, and make adjustments when clinically appropriate. We'll also address any new concerns, answer your questions, and ensure your treatment plan continues to align with your goals. Every visit is collaborative, individualized, and designed to provide the time needed for thoughtful, high-quality care.
